This is a form for a student who is medically injured during the school year. It allows students, parents, and guardians  to communicate with their phyisicians to see what they are able to do during physical education class. Some doctors may allow light exercises, but some may not allow anything. Fill out this form and return it to your physical education teacher when necessary.
